Diagnosis and treatment monitoring of vitamin deficiency

What does a high vitamin B12 result mean?

High B12 is usually due to supplements or injections and is generally harmless; in people not taking supplements, the cause may be investigated.

  • B12 supplements, multivitamins or B12 injections
  • B12-fortified foods and energy drinks
  • Liver disease (B12 released from the liver into the blood)
  • Kidney failure
  • B12 bound to antibodies in the blood (macro-B12), a falsely high result
  • Less commonly, certain blood disorders or cancers

What does a low vitamin B12 result mean?

Low B12 means your body isn't getting or absorbing enough of this vitamin; it can cause fatigue, numbness and anemia.

  • Diet low in animal foods (vegan, vegetarian)
  • Reduced stomach acid in older age
  • Long-term use of acid-reducing drugs (PPIs) or metformin
  • Pregnancy (B12 levels can naturally decrease)
  • After stomach or weight-loss surgery
  • Bowel diseases such as celiac or Crohn's disease
  • An autoimmune condition that blocks absorption (pernicious anemia)

My result is outside the reference range. What should I do?

For borderline results, your doctor may order additional tests (homocysteine, methylmalonic acid). Always tell your doctor about any vitamin supplements and medicines you take.

Reference ranges depend on the method, age and sex; always use the range printed on your report. This information is for general guidance only; consult your doctor for diagnosis and treatment.
