What is this test?
Total iron-binding capacity (TIBC) reflects the blood's capacity to carry iron. It usually rises in iron deficiency and falls in iron overload and some chronic diseases. Together with serum iron and ferritin, it helps determine the cause of anemia.

Differential diagnosis of iron deficiency anemia, iron overload, iron malabsorption
What does a high iron-binding capacity result mean?
A high TIBC means your body is trying to carry more iron and is most often seen in iron deficiency.
- Iron deficiency (the most common cause)
- Pregnancy, especially in later months
- Birth control pills or estrogen-containing medicines
- Heavy menstrual bleeding or blood loss
- Some liver conditions such as acute hepatitis
What does a low iron-binding capacity result mean?
A low TIBC may be linked to chronic inflammation, poor nutrition or iron overload.
- Chronic infections or inflammatory diseases
- Malnutrition or low protein intake
- Iron overload (hemochromatosis, frequent transfusions)
- Advanced liver disease such as cirrhosis
- Protein loss through the kidneys (nephrotic syndrome)
- Less commonly, long-term serious illnesses such as cancer

TIBC is most useful alongside serum iron and ferritin, helping to identify the cause of anemia. Tell your doctor if you are pregnant or taking hormone-containing medicines.
